Patient Consent for Use of Terms and Conditions

NorthShore University HealthSystem is pleased to offer you information via a web-based version of our computer systems called NorthShoreConnect. If you are interested in participating in this service, please read, sign and return this consent form indicating that you have read this Terms & Conditions Statement and agree to it. NorthShore University HealthSystem is a single corporation which includes Evanston Hospital, Glenbrook Hospital, Highland Park Hospital, Skokie Hospital, NorthShore University HealthSystem Medical Group, and NorthShore University HealthSystem Home Services. We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines NorthShore University HealthSystem's practices and our sensitivity to your right to privacy. NorthShore University HealthSystem reserves the right to revoke access of at any time for abusive use of the system.

Response to Electronic Communication - NorthShore University HealthSystem will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available, so a patient should allow at least 24 business hours for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically and the patient should contact 911 immediately. Furthermore, with respect to any electronic communications sent by the patient, NorthShore University HealthSystem is only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, NorthShore University HealthSystem will be unable to provide accurate and reliable services.

Website Links - NorthShoreConnect may offer links to related medical websites not managed by NorthShore University HealthSystem. These website link(s) are for a patient's informational purposes only. NorthShore University HealthSystem does not endorse and has not verified the accuracy of the information in/on these websites, and the patient should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

E-mail and Text Messaging Privacy - Patients who are users of NorthShoreConnect should be aware that they will be notified via e-mail and/or text message when there is new medical information to be viewed on NorthShoreConnect. This means that any person with access to a patient's e-mail and/or text messages will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's e-mail account and/or text messages. Although no private medical information will be sent, the notification that new medical information is available by accessing NorthShoreConnect may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address and/or phone number. Please know that if you send us an e-mail communication, it may be shared with the NorthShore University HealthSystem clinical staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on NorthShoreConnect will be accessible only to appropriate NorthShore University HealthSystem clinical staff.

Security and Confidentiality - NorthShore University HealthSystem affords the same degree of confidentiality to medical information stored on NorthShoreConnect as is given to medical information stored by NorthShore University HealthSystem in any other medium. NorthShore University HealthSystem is committed to protecting the confidentiality of your medical information. We limit NorthShore University HealthSystem employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. NorthShore University HealthSystem shall identify the records released and note the time and date of access each time a patient accesses NorthShoreConnect. NorthShore University HealthSystem has taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. For other than general information viewing, NorthShoreConnect must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Internet Explorer versions 8.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4). You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to NorthShore University HealthSystem by viewing the digital certificate. This certificate verifies the connection between the NorthShore University HealthSystem server's public key and the server's identification. User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a NorthShoreConnect user, you agree to maintain the security of your medical information by: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Family Access (Proxy) Settings - NorthShore University HealthSystem offers three types of family access (proxy): pediatric, teen and adult. Pediatric Family Access (Proxy) Pediatric access allows parents to access the electronic medical record (including HIV & Genetic Testing information) of their minor under the age of 13. Access may be requested by the parent or legal guardian. Teen Family Access (Proxy) Teen access allows parents to access the electronic medical record of their minor between the age of 13 and 18. To ensure that the teen retains some privacy, as required by law, access is restricted to Inbox/Messaging the PCP, Customer service messaging, Immunizations and scheduling appointments. Adult Family Access (Proxy) Adult access allows an adult (over or equal to 18 years of age) to access the electronic medical record (including HIV & Genetic Testing information) of another adult. Access may be granted and requested by the patient, legal guardian, or durable power of attorney for healthcare.
