NorthShoreConnect provides you with convenient web-based access to the healthcare information that's most important to you. You can use NorthShoreConnect to review key information from your health record, to communicate with your physicians, to schedule and review appointments, and to pay medical bills online. NorthShoreConnect is designed to be easy to use with a minimum of computer knowledge, but it may help to remember a few key points about using NorthShoreConnect.

Access to NorthShoreConnect is limited to authorized users. Ask your care team about setting up a NorthShoreConnect account directly, at your next office visit. When creating your account from a personal computer, you will need an access code (provided by your physician's office), your social security number and your date of birth to activate your account. After you activate your account, you will create a username and password that you will use to sign in and to review your personal health information. To sign in, enter your username and password in the "Sign In" window on the home page and click "Sign In."

  1. I am new to this website and I would like to gain access to NorthShoreConnect, but I don't know how.

    There are two different ways you can request an access code. You can have an access code generated the next time you visit your physician's office (no appointment is necessary). You can also request an access code by clicking here. Once you have filled out the form, you can either mail or fax the signed copy back to your physician's office. An access code will then be mailed back to the address we have for you on file. Alternatively, ask your care team about setting up a NorthShoreConnect account directly, at your next office visit.

  2. My doctor provided me with an access code, but I am unable to create my account.

    1. Make sure you include the dashes in the access code and social security number
    2. Check that your date of birth is in the following format: mm/dd/yyyy
    3. When establishing your username, it must be at least 6 characters long
    4. When establishing a password, make sure to use a combination of at least 6 letters and numbers - a mix of both is required to create a valid password
    5. When entering your information, please remember not to include any spaces

  3. I received a message saying that my access code was expired, how do I create my account?

    For security reasons, access codes expire after a predetermined amount of time. You can contact the NorthShoreConnect support line at 847-425-3900 to receive a new access code.

  4. My doctor gave me an access code, but I don't know what happened to it. What do I do if I lost my access code?

    For lost access codes, you will need to contact the NorthShoreConnect support line at 847-425-3900 to have a new access code generated.

  5. I created an account and have logged in before, but I can't remember my password.

    To retrieve a password that you previously established, you can use our forgotten password module. You will need your username, social security number and date of birth. Once those are verified, you will need to know the answer of your secret question. When you have answered all of the questions correctly, you will be able to reestablish a password for future use.

  6. I created an account and have logged in before, but I can't remember my username.

    If you are unable to remember your username, you will have to have your account reset. You can do so by contacting our NorthShoreConnect support line at 847-425-3900.
